Bridge Care Connections offers limited scholarships to reduce or cover costs for eligible clients on Medicaid or who are uninsured. We believe that financial barriers should never stand in the way of high-quality health equity services and restorative care.

• At Bridge Care Connections, we understand that barriers don’t always come in obvious forms. Sometimes it’s financial. Sometimes it’s timing, access, or simply not knowing where to start.

• We created our Access Scholarship Program because we’ve seen firsthand how one barrier can prevent someone from receiving the support they need.

• While our resources are limited, our intention is not. We are committed to reducing barriers where we can—whether that means providing a free resource navigation session for an individual or offering discounted training to small organizations working directly within their communities.

• Every request is reviewed with care, because we recognize that not every situation fits into a predefined category.
   

Scholarship Benefits 

Eligible participants may receive one of the following services once per calendar year:

Option 1: Free Healthcare Navigation Workshop

• Health insurance and Medicaid education

• Understanding healthcare benefits

• Appointment preparation strategies

• Resource and referral guidance

• Healthcare system navigation skills

Option 2: Free Case- Management Consultation 

• One 15-minute individualized consultation

• Assistance identifying community resources

• Healthcare navigation guidance

• Insurance and Medicaid support

• Referrals to appropriate services and programs

Eligibility:

• Applicants must meet at least one of the following criteria:

• Currently enrolled in Medicaid

• Uninsured or underinsured

• Experiencing barriers to accessing healthcare services
